Participant Registration Form

This form is designed to collect personal information about the participant registering to attend the Aboriginal Sport and Recreation Circle NL's Event.

If you are filling out this form for multiple people, you must submit a separate form for every person registering.

Information collected is kept secure under digital password protection and physically under lock and key. This form's information is only viewed by ASRCNL staff for the purpose of organizing and hosting events. Anonymous demographic information such as gender, age, community/region and ancestry is regularly compiled and reported to the ASRCNL's Executive and Board of Directors and its funders. The purpose of this reporting is to evaluate the impact of the ASRCNL's services.

Medical/diagnosis concerns and/or directions that leaders of the event should be made aware, in the event of an emergency.

Are there any allergies the leaders should be aware of to ensure other participants do not bring in any items that may cause an allergic reaction? Ex: Latex. Balloons are sometimes used for shooting but we can adapt accordingly.
